Students entering a Miami - Dade school for the first time must have a parent or legal guardian present at time of registration with the following documents:

Verification of Residence:

  • Must bring a Florida Power & Light bill in parents or legal guardians name
  • Notarized lease
  • Mortgage
  • Deed to residential property
  • A letter from leasing agent or office on official letterhead indicating residence
  • If the above documents cannot be obtained, a visiting teacher will be sent to verify residence on behalf of the school

Students Records:

  • Copy of most recent report card and academic history from previous school
  • Withdrawal grades
  • Standardized Test Scores
  • Birth Certificate or Passport for the child
  • Social Security number (not required, but can be provided)

Medical Forms:

  • Florida Physical with TB Skin Test (Form 3040)
  • Florida Certificate of Immunization (form 680) to include:
    • DTP
    • Polio
    • MMR ( Measles Vaccine two doses)
    • Hepatitis B (Three doses)
    • Tetanus - Diptheria (TD) Booster

Students transferring from another Dade County School must have the following:

  • Transfer slip
  • Must bring a Florida Power & Light in your name and one of the following:
    • Notarized lease
    • Mortgage
    • Deed
